Dental Insurance: We know that understanding your dental insurance coverage can be quite difficult. Our goal is to assist you in maximizing your annual benefits. We accept dental insurances; however, we are not contracted as preferred providers with all of them. We recommend that you contact your insurance company to request a list of their preferred providers. Each dental plan provides different levels of coverage and allows for specific procedures. We strongly encourage you to become familiar with your policy exclusions, deductibles, maximums, and required co-payments. It is your responsibility to know how your coverage works.
As a courtesy to you we will electronically submit your insurance claim with any necessary x-rays and request that payment be sent directly to us.
We will estimate your out of pocket expenses to the best of our ability. Please remember that this is just an estimate based on the insurance company's Usual and Customary Rates (UCR). This is not a guarantee of payment from your insurance. All restrictions are based on the premium paid for the insurance, not our fees or recommended treatment.
Our expectations of you as the owner of the policy:- Payment of all fees not covered by your insurance plan at time of treatment.
- Please understand that the insurance policy belongs to you and we have no leverage to obtain payment from your insurance carrier.
- You will be responsible for any remaining balance left unpaid by the insurance. This balance is due immediately.
- You will need to bring your insurance card as well as your insurance coverage book with you to your first appointment. This will assist us in billing your insurance. If you do not have either of these, please contact your employer. They are required to provide you with each of these items.
